What is Included in my Personal Plan?

Our 6 month membership is a way for you to stay accountable, committed to yourself, and evolve your wellbeing alongside a team dedicated to support you including:

  • 4 visits with our Functional Practitioner/Clinical Nutritionist, using nutrition as the key overlooked piece.

    • Begin with an in-depth intake questionnaire summarizing your goals, current symptoms and experiences.

    • Follow up with an initial consultation so you can tell your story and be heard in an un-rushed appointment together.

    • Go over your personalized plan to work on over the course of your program.

    • 3 follow ups to follow you along in your progress and adjust your plan for optimal wellbeing.

  • Group Coaching sessions with your coaching team to:

    • set intentions and focus and get support to implement strategies that support them.

  • Personalized nutrition and lifestyle plan to address all components of well being.

  • Access to advanced assessments for an additional cost

  • Access to supplements at an additional cost

  • Lifestyle, food and mood journals to help you become self empowered as well as staying connected to your team.

  • All in one dashboard to access your education, your plan, your sessions and appointments and message your team.

  • Progress tracking with our monthly symptom inventory

  • Access to a self paced educational programming system designed to give you the science behind, and the how to implement new strategies along with resources, toolkits and tips for your improved wellbeing.

What’s Not Included in my Personal Weight Plan?

Some costs that are not included in your membership are decided by you and are meant to be an additional benefit to your membership. These include:

  • Advanced assessments

  • Supplements and meals

How Much Does My Personal Weight Plan Cost?

Our annual membership to help you integrate a lifestyle of wellness activity in your daily life. Choose from 3 options to fit your perfect need, ranging from $118/month to $241.67/month.

Does Insurance Cover My Personal Wellness Plan?

Not yet! But we are in the works! You can always reach out to our office for options and/or payment plans that may be better suited to your needs.

How Do I Cancel My Plan?

Memberships are one way we at ANMC deliver vitality support to normalize wellness and wellbeing, set as an annual plan, because we know real change takes time. If you are concerned that you will not be able to utilize your 12 month membership, you should consider the other options available to you ahead of time by reaching out to our office.

If you ARE ready, however, with the best intentions to follow through for at least 12 months, join us! We understand things happen and if you find you are unable to continue your membership, please request cancellation by messaging your team through your dashboard. We will then process one final membership fee during which you will have access to your membership services until the end of that billing period. If you paid upfront, you will not be charged a final fee to cancel and you will have access to your materials through your cancellation date.

Due to the digital nature of our educational materials, we do not allow refunds once digital products are delivered at sign up.

What Does “Unlimited Messaging” mean?

As a member, this is a great way to get your questions answered if it can’t wait until your next session.

NOTE: Unlimited messaging is NOT for emergency care. If you require medical attention place call 911 or visit an urgent care center.

What Does My Personal Plan Look Like?

Week 1: Getting started:

  • Complete your initial intake

  • Schedule and have your initial functional consultation with your clinician, for those plans that apply

  • Schedule your upcoming appointments after this initial. Appointments are encouraged to be scheduled all at one to establish your spot.

Month 1: Meet with your Coach and Go Over Your Personal Plan

  • Set strategies and focuses to work on until your next session.

  • Attend and introduce yourself at Group Sessions, if available in your plan.

Monthly:

  • Each month schedule and have a Body composition assessment, included in the House Clinic plan

  • Utilize the Symptom Tracker that will be sent to you monthly to track your progress.

Ongoing:

  • Utilize your available lifestyle and food/mood trackers to see how well you are doing and support your efforts.

  • Review the next module available in your Programs from your Dashboard to continue educating and empowering your wellness evolution.

  • Attend your Group Sessions weekly

**Results are not guaranteed. As an educational, accountability and self-sustaining program, results will depend greatly on the compliance and effort you invest into yourself.
